http://www.moneris.ca/ in the past, and can highly recommend them. Excellent company. Then again, it's owned by the two largest bank in Canada, so go figure.
Easy application process, about $700 to get setup with both VISA & MC, excellent customer service, funds are within your bank account usually 48 hours after a card is charged, etc. They're great!
Not for adult though, but for mainstream.
