When his kids run up to him, hes son says "look what I've been building" and then something else I couldnt make out. That tells me that they can build things in the sand that take shape in the dream world.
So the first thing that popped into my head at the end this time was that Cobb is in his kids dream world at the end. Somehow they (maybe with the help of Caine's character) built a dream world from Cobb's last memory of them. Why couldnt Michael Caine bring his kids to see Cobb in another country? It could even be possible that Cobb had died. Remember when his wife stabs him at the end, last time we see her. What happened to him there? There was no effect to him after Moll is shot by Ariadne. Just throwing that out there.

In his kids dreamworld....... Now THAT would be a twist. Did he actually show him what he had been building or just say it?
I don't think he died from the stab, or dead at all. But I do think he is stuck in a dream world somewhere.
Shit, maybe he is still in limbo. They did say anything is possible there and you create it as you go. Maybe Salto shot himself, woke up and moved on while Cobb did stay behind in limbo because he knows the truth that he can not return for some reason OR there was only one bullet left and Cobb could not get a kick.
This movie is a pure work of genius.
