It's fresh brewed sweet tea at all the ones here in NY, no mix or shit like that. I've seen them making it with hot water and tea bags.
That said, it is generally very sweet, so I have them mix 1/2 and 1/2 with the unsweetened tea.
And you have to ask for the lemon...they have 'em, you just have to ask
