I would actually classify it as 2 niches. Emetophilia is when someone just gets off from the act of puking. I don't think it has to be in a sexual context at all.
I would consider puking or gagging from blow jobs an entirely different niche though. Rough face fucking is a niche on it's own and sometimes puking just comes with the territory, but I don't think puking is the main attraction.
