I don't know how old you are, but it sounds like you have an enlarged prostate. You exhibit the classic symptoms. The prostate is located under your urethra, so when it gets enlarged, it presses up against the urethra and starts to obstruct the flow of urine. This would explain why you are not urinating as often, and is also indicative of the deep-seated pain that you are having.

Most men experience a prostate enlargement once they grow older, rarely it is the result of cancer. If it is enlarged, the next step is to get a PSA test and then a biopsy to determine if it is cancerous.

Either that or it could be a Urinary Tract Infection with some sort of Uropathogenic E. coli. If antibiotics haven't been able to clear them up, they probably are the type that forms a biofilm, which could also explain the urethral obstruction. I would suggest that you do not take any antibiotics until consulting with your doctor. If it is a bacterial infection, taking antibiotics once isn't going to have an effect on the bacteria, if anything it will lead to increased resistance.
