The difference between "gonzo" and "wall to wall" can be encapsulated in two words: TALKING CAMERA. In gonzo, the cameraman -- yes, man, if there are any women shooting gonzo they don't come to mind right now -- talks and is actively involved in the scene to the point of commenting and sometimes participating as well.
The term was devised by Gene Ross when he was at AVN to recognize that kind of porn that was real, much like Hunter S. Thompson's stream-of-consciousness gonzo writings tried to achieve in print.
Gene could illuminate further on the origins of the term, if somebody asks him.
