I would burn through it within an hour or so, and get that "crash" where you feel really tired. I tend to metabolize all drugs quickly, so my doctor and I figured out that the solution was to either go with the extended release (which is expensive as fuck due to lack of a generic version) or just take the lowest dose possible more frequently. That worked, so I stuck with it.
If you don't feel any difference, it might not be the right treatment for you. Adderall is not like an antidepressant where you have to continue use for weeks before you feel results - it should be immediately apparent if it's going to help (thus why some people abuse it for energy or to help them study for an exam or just to get high, because it can be taken at random for the same effect without having to build up in your system).
Have you tried any other drugs? You should ask your doctor and see if he'll try putting you on either a different dosage/extended release or a different drug entirely.
ETA: I used to work in a hospital pharmacy for a few years before getting into running sites, so forgive me if I word-vomit about Adderall =P
