We have one that was made inhouse a couple of years ago
1) ftp upload video & thumbnail
2) Play video file in a small pop-up window
3) Video download link (not a direct link)
4) File information such as video height, width, size, runtime will be added by the script
5) Search function for members (title)
PHP, MySql backend, the script is setup to play quicktime & wmv files.
