Supermodels are usually the highest paid in the fashion industry. The ones that got the million + contracts from the cosmetic or clothing companies. The title meant something awhile back when only a select few received such. Today the title is thrown around so much and self proclaimed that it is practically meaningless.
Centerfolds are usually the " featured model " who's pictorial appears in the middle of the magazine with a single photo, two page spread spanning across the exact middle where the staples were exposed ( back when magazines were stapled and not bound like today ) so you had the center of the magazine where it folds together hence " centerfold " Certainly the term was made famous by Playboy and I believe they coined the term as well.
Every nude model is not a centerfold. That title goes to the model that has graced the the center fold of the magazine it it's feature pictorial. Just like " Covermodel " is only for those models that have appeared on a magazine's cover.
As someone has already pointed out Supermodels make a zillion more than Centerfolds.
Just wanted to add that a model can be anyone that wants to be reproduced in some kind of medium ( video, picture, painting etc )
