Jiri Fischer of the Detroit Red Wings comes to mind. He didn't die, but he collapsed on the bench during a game back in 2005. He's still on the team, but hasn't played since the incident, except to play in a charity hockey game.
Jiri Fischer collapses at game
It happens around the 1 min mark:
and the continuation of it:
Here's a good write-up on it, a year later, in which Jiri actually states,
"I died. I died and I was brought back."

"There was no pulse, no heartbeat, and with emergency technicians and team trainers helping out, Colucci cut away Fischer's jersey and pads and administered an AED (automated external defibrillator) to shock Fischer's heart back to life."
Scary shit. Sadly, you can never predict when something like this will happen. That last article on Jiri says that if he had collapsed anywhere other than the Joe Louis Arena, he'd be dead. Being in Russia, where they may not have as many trained medical staff on hand, obviously did have a bearing on poor Alexei's life.
