Oh yeah, I've lost a lot of money on ads that didn't work.

The worst ones are blogs and tubes. They charge an enormous amount and don't make enough ROI.

One thing though I'll say in the defense of under producing ads is two things:

1. We are VOD and with a 10 free minute option and being non-recurring that does hinder pre-paid ads in comparison to normal paysites.

2. People must be willing to optimize their campaign continuously. If a banner doesn't work, switch it out, play with text.
