When I got married (years ago) you had to provide the DMV and Social Security office with a notarized copy of your married license. You also had to notify credit card providers, mortgage brokers, car loans ect. I dont remember if you had to just notify your bank or present them also with a copy to change your name on your bank account. And of course notifying your employer so your information matches come tax time.

I think that's it and I remember it being a huge pain in the ass which is why I never changed my name back when I got divorced.
