Copy of Credit Card Statement
Completed Form
Proof of Name and Address (utility bill)

I do have questions:
1. I have emailed epassporte about 12 times over the years. In total I received only 1 reply which was responded to 3 months after I emailed. How will epassporte be able to process all of this information when they can't keep up with their current workload?
2. As I am sure you are aware, terms have different meanings to different people. I keep my jewelery in a "secure location", which is a safety deposit box at my bank. Others might consider a jewelery box with a lock on it secure, or under their mattress secure. What exactly does "secure" mean for epassporte? Does that mean a bank vault? Does that mean a locked filing cabinet in an open area? Does that mean an unlocked filing cabinet in an office with a standard easy to open lock?
3. Why could you not have told people to black out personal information PRIOR to asking for it? What about all of the people who have already breached their personal security to satisfy your request...how will they feel knowing they did not have to do this?
4. How do we go about removing load cards from our account? I read where someone no longer had their load card and your suggestion was to load another, what if people do not want to have a load card at all anymore?
5. What is epassporte going to do with all of the information requested, and then uploaded by people that you say they do not need (credit card numbers etc.) Will epassporte have a bonded individual blacking out this sensitive information prior to handing it out to the minimum wage club where a frenzy of personal information could set off an urge to steal people's information?

You need to send in the documents asked for in the form:
A photocopy of a valid government-issued photo ID
A copy of front and back of the credit/debit card most recently used on your ePassporte account
A copy of the most recent statement of the credit/debit card used to load funds to your ePassporte Account,
or a copy of a utility bill if you do not have the credit/debit card statement with your name and address as shown on your ePassporte Account clearly displayed.
So either a statement or utility bill is required.
1) If you have not previously uploaded the documents through the document upload system you will need to submit the documents again.
2) I will find out if I can go into details about this.
3) Full documents are preferred if you are uncomfortable with this we will on a case by case basis allow documents with information blacked out.
4) If an account is opened with a credit card or an account attach a load card a valid credit card is required to be on file. Used load cards are kept on file in case if future CBs for example.
5) We have to keep the information on file for anti-money laundering/know your customer reasons any information will only be given our with a subpoena or court order, all documents are handled by trusted staff.
