From what I understand, the plyers came into play when they were trying to remove something else that got stuck up there and then the plyers got lost as well and by the time they arrived at the ER, they were so far up there that they were afraid if they just "extracted them" by yanking them out that it would cause damage, so they went on a surgical recon mission to remove them.
