the woman eatting the cake is content for a fetish called 'feederism'. It is something I (and a lot of other people in the BBW community) have a great deal of trouble with. The point is that the feeder (usually a man) has his feedee eat and eat to make them bigger (often 'forced'). Some just like them to get a bit bigger..others want the women to reach the level of having mobility problems and being dependent on the feeder. It is the dark underworld of the BBW community for the majority who would never want to harm the health of their partners for a sexual kick.
