Well we are not in the middle age, working girls as I call them aka prostitutes try to care about them as much as possible, they don't perform anything without a condom and most of them are using all kinds of lotions or different stuff to keep them desinfected all the time.
We for example also have a very strict anti dope policy with regular testing, so overally I would be way more worried about average level pornstars then high level prostitutes.
