Anything apart from that, though, would be quite fucked up. Imagine two kids with cancer, one getting worse medical treatment than the other because his parents don't have enough money to pay for the best quality. A sickening thought

Well, we can pay extra for private rooms (if any are available) and things like that but we can't pay directly for medical or diagnostic procedures.
