Saw this on a Soprano's board and this is my favorite explanation.
if Chase meant for us to think Tony got popped he would have made it so that before he looks up to see Meadow coming in we would have seen the limp dick with the members only jacket coming out of the bathroom from the side or given some kind of hint that, that was even a possibility. The point chase was making was for the rest of Tony's life he will not be at ease, always worried if the next person coming through the door was coming to whack him. Chase made us all feel nervous as hell with every last person who walked in the door. Just as Tone now will. Every time the bell rang Tony's head popped up and it shows that he was just as nervous as we were.
