There is no fee to get a paypal account. Put your credit card, and bank account online. It will say confirm your credit card by waiting on mail but the bank will usually be confirmed the next day and then the account works anyway.
As for the affiliate script just do a google search for it. I think there are several available.
As for the fee, some people have been charged a fee for doing adult websites. I did one for a very short time and never got asked about the fee. You just set up a subscription button. You may want to call first but some of their reps are definately clueless.
