I know one of our top affiliates used
http://www.sexxxydesign.com/ for his private label and was very happy with their work. They understand hwo to work with our templates.
I would personally also recommend
http://www.bluedesignstudios.com as they have done some great work for us (no sure if they are familiar with smarty templates but I'm sure they would grasp it quickly).
