Actually, no. People who abuse their organs are usually put at the bottom of the list for donations (unless you can buy one yourself like some of the rock stars). Doctors aren't stupid. They aren't going to put healthy lungs in a 3 pack a day smoker instead of someone who has a hereditary lung disease.
And if you're in the US, I doubt you have the ability to tell them where your organs go. IOW, you're either a donor or not. So....your relative could be at the top of the list for heart donation, but since you marked "NO" for donation, they're out of luck.
