Yes it changes the rate at which the wheel spins relative to your actual speed. There is an equation that they can do to figure out the proper size for a new speedometer gear if its an older car. With newer electronic speed sensors there is sometimes a gear also. If not you may be able to get the computer in the car adjusted so your speed is showing correctly.
