The Fern Grotto on Kauai is beautiful. Also, there is a site-seeing helicopter ride on Kauai that is absolutely spectacular.

Oahu has more of the tourista type things, such as the Polynesian Cultural Center, the Botanical Gardens and such. If they swim, then Hanauma Bay is awesome for snorkling (you only need to be a moderately good swimmer).

I'm sure they'll have a great time regardless. Tell gramps to not forget his prescription of little blue pills...
