I don't know the correct answer as I haven't filled in the form, but I do know that Ibill is trying to get in the good books with Visa as much as possible and telling you to put down the forming director (The US citizen) is giving Visa deceiving information.

I believe that Visa wants YOUR name to run against their terminated merchant file to make sure your not already on it and have been hiding under the cover of a third party processor.

Then I guess they will house your name and details incase they decide to add you to the TMF in the future and this info will then be used to try and prevent you from entering their realms again.
