Unfortunately this convention is not a standard and breaks a great many things, including php - it makes php start the output when the file does not immediately start with <?php - and this is not easy to detect, because when you open this file, you see that it does, because your software removes those two bytes again.
It appears that this is a probable cause of your problems, the only way to fix this is to try another editor which does not follow this silly convention and check your index.php.
