It's because he injects Synthol directly into his biceps to pump them up (hence the bandage in the last pic). Synthol is a synthetic posing oil (not meant to be taken internally) and injecting it is not only stupid but also dangerous. In other words, they don't have big muscles they have inflated muscles.
