I think this line:
RewriteCond %{HTTP_REFERER} 67.18.226.50 [OR]

should be like this:
RewriteCond %{HTTP_REFERER} 67\.18\.226\.50 [OR]

If he's not sending referrer information though, your second half is letting him in with this line:
RewriteCond %{HTTP_REFERER} !^$

Not sure what to suggest really.
