The original mpeg is such an antiquated format that I can't really understand why people would still use it - the story of compatiblity w/ macs rings true less each day - if any of your mac using members ever report issues, tell them to get a copy of .. mplayer :D

As a few people have pointed out, the best way to do it is to do it manually.
For those not familiar w/ the technique, view the video gallery, find its location in the data directory (ie /5/8347fd28df832a738e/) - go to your server - carma/cms_data/[data directory] - replace the sample.jpg with the picture you'd like to be the sample .jpg.
If you're doing this retrospectively, delete the other samples (ie sample120x90jpg.jpg) to prevent these showing up again.
Oh, and all care and no responsibility taken - BE CAREFUL in those directories, and never delete .sample, .sort or .types
